Have you ever noticed an itchy mouth, tingling lips, or scratchy throat after eating fresh fruit—especially during allergy season? You’re not alone. This reaction may be due to cross-reactivity between pollens and certain foods, a condition known as Oral Allergy Syndrome (OAS) or Pollen-Food Allergy Syndrome.
Cross-reactivity occurs when your immune system mistakes proteins in certain foods for pollen allergens because their structures are very similar. If you’re allergic to pollens like birch, ragweed, or grass, your body may react when you eat certain raw fruits, vegetables, or nuts.
Here are some typical examples:
Birch pollen: apples, pears, peaches, cherries, carrots, celery, hazelnuts
Ragweed pollen: bananas, melons (watermelon, cantaloupe, honeydew), zucchini
Grass pollen: tomatoes, potatoes, oranges, peaches
Symptoms usually appear within minutes of eating and are often limited to the mouth and throat.
Itchy or tingling lips, mouth, or throat
Mild swelling of the lips or tongue
Scratchy throat or hoarse voice
Rarely, more significant reactions
Symptoms tend to be worse during peak pollen seasons, when your immune system is already on high alert.
Heating or cooking foods often breaks down the allergenic proteins, which is why many people can tolerate cooked fruits and vegetables but react to them raw. For example, baked apples may be tolerated even if raw apples cause symptoms.
While Oral Allergy Syndrome is usually mild, you should seek medical attention if you experience:
Difficulty swallowing
Hives or widespread itching
Shortness of breath
Severe throat swelling
These may indicate a more serious food allergy rather than simple cross-reactivity.
Allergy testing can help determine whether your symptoms are due to pollen-food cross-reactivity or a true food allergy. This distinction is important for managing symptoms and avoiding unnecessary food restrictions.
